Information for professionals

For commissioners and social workers

Lexrae Community Hub is community based day opportunity provision in Telford for adults and young people, not a building people are parked in.

What we offer

Day opportunities for working age adults and young people in transition

Community based sessions Monday to Friday, 9am to 5pm. We support under-18s as well as adults, so a young person can start before they move into adult social care and keep the same faces afterwards.

Prevention and delay of higher-cost support

Structured, purposeful days that maintain skills, routine and community connection, reducing the drift towards more intensive and more expensive packages.

Respite that supports unpaid carers

A planned, reliable break, with a written record of the day the carer can read afterwards rather than a phone call they have to chase.

Skills for independence and daily living

Cooking, budgeting, shopping, travel, timekeeping and working alongside other people. Goals are agreed with the person and tracked session by session.

Work experience placements arranged on request

Work experience placements are arranged on request. This is not a supported internship and it is not a standing programme. A placement depends on what an employer will host at the time, and it is not guaranteed. We would rather say that now than have you arrive expecting something else.

How we evidence outcomes

We produce a person centred progress report covering a chosen period. It contains:

  • the goals the person is working on, and how they have moved over time;
  • engagement and attendance, taken from the sessions actually recorded;
  • the person’s own words, where they have agreed to those being shared;
  • photographs, only where consent covers sharing them in a report.

Every statement is drawn from recorded sessions. Anything that cannot be evidenced is left out rather than softened, and there are no invented percentages or success rates anywhere in a report.

Reports are drafted with AI assistance. A member of staff gathers the recorded facts, an AI model turns them into readable prose, and a manager reviews, edits and approves the report before it goes anywhere. Once approved it cannot be altered. We say this plainly because you should know how a document in front of you was produced.

Attendance, activity and outcome information can also be supplied for a funded person on request.

Example, made up, for illustration

Progress report

A. B., 1 January to 31 March. Prepared by the Hub team.

Attendance
Attended 22 of 24 planned sessions.
Goal: prepare a hot meal with less help
Recorded on 14 occasions. Support moved from step by step prompting to a written recipe card and occasional checking. On the last four occasions A. cooked with prompting only at the hob.
In their words
“I like doing the chopping. I want to make a curry next.”
Photographs
Two photographs included, both covered by current consent.
Approval
Drafted with AI assistance from recorded sessions. Reviewed and approved by a manager before release.

Families can see the day

Families and carers with an approved link get their own login. They can see what happened in a session, photographs where consent allows, progress towards goals, and they can message the team.

For you, that means fewer chased phone calls and a family who already know how things are going before a review. Access is controlled per person and approved by a manager, and every link can be withdrawn.

Flexible funding

Local authority commissioned

The council contracts the placement and we invoice the council directly. The person is not asked for money.

Direct payment

The person or their representative buys sessions with their direct payment. We provide an invoice and a receipt for every payment, so spend can be evidenced to the council.

Self funded

Sessions bought privately at our published prices. No card fees and no booking fees.

Lexrae Support Ltd is not registered for VAT, so no VAT is charged. See the published prices.

Governance

DBS and training. Everyone is checked through the Disclosure and Barring Service, and nobody works unsupervised until they are trained.

Consent. Consent is recorded per person and per purpose, so agreeing to a photograph in a care record is not the same as agreeing to it appearing in a report. Consent can be withdrawn at any time and takes effect immediately.

Records. Access is enforced in the database, not in the browser, and every change is logged. Records are retained in line with our policy and then disposed of.

Policies. Our policies are available on request. Ask the named contact below and we will send them.

Complaints. Our complaints route is published, including how to escalate outside the service.

How to refer, and who to contact

Please do not send a full referral through the website form. It is not the right place for detailed information about someone. Telephone or email us first and we will take it properly.

We will arrange a visit, agree what the person wants to work on, confirm what is funded, and set a start date. Transport can be arranged where it is included in the support plan or funding.

Evening sessions from 4.30pm to 9pm and a weekend club are planned. They are not running yet and cannot be commissioned.

Contact for commissioners

We are at Edward James House, Hadley Park, Telford, TF1 6QJ, open Monday to Friday, 9am to 5pm.
